Sliding scale/discounted rates (if needed and approved) are as follows

We currently accept Presbyterian, Blue Cross Blue Shield (both commercial and Centennial Care Plans), Western Sky, United/Optum, Aetna, Cigna, Medicare plans and Molina.

We currently accept the following insurances for medication management: BCBS (commercial and medicaid plans), Presbyterian (both commercial and Centennial plans), United, Aetna, Western Sky, Cigna, Medicare plans and Molina.
Regardless of insurance, you agree that you are responsible for payment of all fees for services rendered.
Cash, checks and debit/credit cards are acceptable methods of payment and a receipt will be provided upon request.
If you are unable to attend a session, please make sure you cancel at least 24 hours in advance. Otherwise, you may be charged a no-show/late cancellation fee.
